The Most Important Part of Regenerative Medicine Happens Before the Injection
Why preparing your body may be just as important as the biologic treatment itself.


Most people think regenerative medicine begins on the day of the procedure.
The blood is drawn.
The platelets are prepared.
The injection is performed.
Then the healing begins.
In reality, some of the most important work should have started weeks earlier.
PRP and bone marrow aspirate concentrate, or BMAC, provide biological signals intended to support tissue repair and recovery. But those signals do not enter an empty space. They enter a living system shaped by muscle strength, joint stability, movement patterns, metabolic health, sleep, nutrition, inflammation, and prior treatments.
That environment matters.
A carefully prepared biologic treatment placed accurately into the correct structure can still be asked to work against weakness, instability, poor loading mechanics, or a body that does not have the resources required for repair.
The injection may have been performed well.
The biology may have been appropriate.
But the system receiving it may not have been ready.
The Injection Is Not the Entire Treatment
Patients are often told to prepare for PRP by drinking extra water, eating breakfast, and avoiding certain medications.
Those instructions may be useful on procedure day.
But that is not prehabilitation.
That is scheduling.
True prehabilitation asks more substantial questions:
Is the region mechanically stable?
Are the muscles surrounding it strong enough to manage load?
Are movement compensations continuing to irritate the tissue?
Does the body have adequate protein and nutritional support for recovery?
Are sleep disruption, metabolic dysfunction, or chronic stress interfering with healing?
Has the diagnosis been confirmed, and is the proposed injection targeting the structure actually responsible for the problem?
These are not peripheral wellness questions. They help determine the environment into which the biologic treatment will be delivered.

Preparing the Whole System
Prehabilitation is not simply exercising before a procedure.
It is the physician-directed process of preparing the biological, mechanical, and neurological environment before regenerative treatment is delivered.
That preparation may include:
Mechanical readiness
Restoring strength, stability, movement quality, and appropriate loading around the joint or spinal region.
Biological readiness
Reviewing metabolic health, inflammation, nutrition, tissue quality, medications, prior injections, and factors that may influence healing.
Neurological readiness
Addressing sleep, chronic pain processing, stress physiology, and the communication between the nervous system and the muscles responsible for stability.
Diagnostic readiness
Confirming that the symptoms, examination, and imaging point to the same target before any treatment is planned.
Not every patient requires the same preparation.
A knee with advanced arthritis has different needs than a shoulder with scapular weakness. A painful disc and an unstable spinal segment require a different plan than an isolated tendon injury.
That is why prehabilitation should be a system, not a generic checklist.
Technical Excellence Still Matters
Preparing the patient does not replace the technical standards of regenerative medicine.
The biology still needs to be appropriate.
The dose still matters.
The target still matters.
The injection still needs to be delivered accurately under ultrasound or fluoroscopic guidance.
But even excellent biology, delivered at an appropriate dose into the correct location, is only one part of the clinical equation.
The goal is not simply to perform an injection successfully.
The goal is to give the treatment the best environment in which to do its work.

Why Your PRP or Stem Cell Therapy Did Not Work
A disappointing result does not always mean regenerative medicine cannot help.
Sometimes the diagnosis was incomplete. Sometimes the dose or delivery was inadequate. Sometimes the correct biology was placed into a body or mechanical system that had not been prepared to receive it.

Supporting the Building Blocks of Recovery

Regenerative medicine should not begin on procedure day.
It should begin with determining whether the diagnosis is correct, whether the treatment is appropriate, and whether the body and the mechanical system are prepared to support recovery.
The injection matters.
What happens before the injection matters, too.
